Motion For Entry Of Default
This form is used by the plaintiff to ask the court to enter a judgment of default against a defendant who failed to answer, appear or otherwise respond to the plaintiff’s complaint and summons within the time limits prescribed by law.

This motion for entry of a default judgment against the defendant is filed with the court by the plaintiff. The original document goes into the court case file, and a copy is given to the defendant.

Complaint For Absolute Divorce
A Complaint for Absolute Divorce starts the process for an uncontested divorce. This “divorce complaint” is filed in district court, in the county in which either the plaintiff or the defendant resides. Once separated for a year, either party can file for an action of absolute divorce.

Verification
This is a document signed by the plaintiff that states that all of the statements and facts in the Complaint for Absolute Divorce are true to the best of the plaintiff’s knowledge. The plaintiff will need to sign and date the form in the presence of a notary public.

Affidavit Of Service By Certified Mail
This form is a statement made under oath by the plaintiff that the defendant was served by certified mail. When an item is sent by certified mail, the post office sends a green card back to the sender, once the recipient has signed for the piece of mail.

The green card must be attached to the notarized Affidavit of Service By Certified Mail when the affidavit is filed.

Certificate Of Service
